1 The role of peripheral afferents in persistent inguinal postherniorrhaphy pain: a randomized, double-blind, placebo-controlled, crossover trial of ultrasound-guided tender point blockade  N Wijayasinghe, T.K. Ringsted, J.M. Bischoff, H Kehlet, M.U. Werner  British Journal of Anaesthesia  Volume 116, Issue 6, Pages (June 2016) DOI: /bja/aew071 Copyright © 2016 The Author(s) Terms and Conditions

2 Fig 1 Trial algorithm. Identical seven-day pain and sleep diaries (Questionnaire 1–3: only patients) were completed before Session 1, between Session 1 and 2, and, after Session 2. After baseline assessments of pain (summed pain intensity [P1/P3]: at rest [supine], on movement [supine to standing] and with pressure algometry [100 kPa]) and Sensory tests 1/3, an ultrasound-guided blockade with placebo or bupivacaine (Block 1/2) was performed. Thirty min (arrows) after induction of the blockade repeat assessments of pain (P2/P4) and Sensory tests 2/4 were performed. The sensory tests included thermal detection and pain thresholds, and, supra-threshold heat pain perception and pressure pain thresholds. Yellow triangle at the top of the image represents the middle of the ultrasound probe, which is placed on the tender point. a: before injection b: during injection; visualization of spread of the solution (black area). 5 Fig 4 Dot-line diagram presenting summed pain intensity difference (SPID) after placebo and bupivacaine injections for individual patients. SPID is expressed as a percentage and calculated as: ((SPIpre-injection–SPIpost-injection)/SPIpre-injection)×100. 6 Fig 5 QST results for all participants. All Δ values are post-injection – pre-injection values. WDT, warmth detection threshold; CDT, cool detection threshold; HPT, heat pain threshold; STH, suprathreshold heat pain perception; PPT, pressure pain threshold. For calculation of WDT, CDT, HPT, STH and PPT see Methods. British Journal of Anaesthesia  , DOI: ( /bja/aew071) Copyright © 2016 The Author(s) Terms and Conditions
