What are Life-Saving Rules?
Why established To increase awareness and understanding about the rules, while providing consistency in how they are shared, applied and enforced across all Mosaic Phosphate facilities Basis Not new Based on existing safety standards, programs and procedures that address Phosphates’ areas of greatest risk and activities that have the highest historical potential for life-threatening incidents Workers are empowered and accountable for the safety of themselves and others – which is crucial to the successful transformation of our business.

What does this mean? We could have prevented.... 63 of the 82 serious or potentially-serious incidents we had in 2018 if we had followed existing safety policies and procedures covered under the Life-Saving Rules.

Hazardous Energy Control
1 Life Saving Rule – I will follow Lock-Out, Tag-Out procedures and verify isolation of all hazardous energy before work begins. Description Lock out/Tag out/Try separates workers from hazardous energy such as electricity, pressure, hazardous materials, toxic/flammable gas, chemicals, hot liquids, radiation or stored potential energy. This means the equipment is de-energized, isolated, locked and tagged, and verified for zero energy before work begins. Examples Working on equipment such as pumps, MCC’s, line breaks, rotating equipment, etc. Stored energy such as that contained in springs, counterweights, steam or water pressure, etc.

Potentially Hazardous Work
2 Life Saving Rule – I will verify that all controls are in place before completing hazardous work. Description Potentially hazardous work requires additional controls to safely complete the job. This includes work such as line break, hot work, trenching, work around water, etc. Examples Line break/Equipment Opening – Disconnecting piping or opening process systems that could or have contained hazardous materials (phos acid, sulfuric, steam, ammonia, etc.) by unbolting, unscrewing, cutting, or other means Hot work – welding, cutting, soldering, etc.

Confined Space Entry 3 Life Saving Rule – I will obtain authorization before entering a confined space. Description A confined space such as a vessel, tank or pipe may contain explosive gas, hazardous atmospheres, or other hazards such as lack of oxygen or overhead buildup. Confined spaces require a valid permit including atmospheric testing. Examples Entering vessels, tanks and spillways Covered rail cars, tank cars and trucks Dragline tub compartments

Working at Heights 4 Life Saving Rule – I will only use approved fall protection equipment and will always be properly tied off whenever there is a fall potential of 4 feet or more. Description If there is a fall potential of 4 feet or more, approved fall protection equipment appropriate for the fall hazard must be worn. Examples On top of rail cars Around unguarded openings In aerial lifts

Barricading 5 Life Saving Rule – I will ensure barricades are erected prior to creation of a hazard and only enter barricaded areas when authorized. I will not leave any hazard unprotected. Description Barricading limits access to at-risk areas and protects workers. Examples Installing or removing process equipment Around trenches or excavations Creating or working near any unguarded opening When overhead hazards are present

Mobile Equipment Safety
6 Life Saving Rule – I will safely operate mobile equipment for its intended purpose, only after obtaining proper training and authorization, including when working around water. Description Safe operation of mobile equipment by qualified personnel is critical to ensure the safety of the operator and other employees in the work environment. Examples Operating cranes, high reaches, haul trucks, loaders and excavators Working around people, other equipment or structures Performing earthwork near water

Lifting Operations 7 Life Saving Rule – I will only operate lifting equipment within safe working limits and clearances, complying with all lifting procedures. Description Using equipment for lifting activities requires detailed planning and strict adherence to lifting procedures by all personnel involved in the lift. Workers are required to receive training to understand the hazards and controls. Examples Installing new vessels or structures Demolition operations Lifting near hazardous material lines Working under or near power lines

Driving Safety 8 Life Saving Rule – I will only operate a vehicle on company business with a valid license, while wearing a seatbelt, operating at a safe speed, and without distraction. Description Safe, responsible operation of vehicles is required to ensure protection of Mosaic employees and our community. This includes driving without distraction, such as non-hands-free cell phone usage, reading of text messages and s, and wearing headphones, earbuds or other similar devices to listen to audio playback. Examples Driving from site to site in company or personal vehicle Operating Mosaic vehicles in mine/plant areas Personnel carriers such as golf carts, ATV’s, etc.

Critical Safety Devices
9 Life Saving Rule – I will NOT disable a critical safety device without a bypass permit. Description Critical Safety Devices automatically mitigate operational deviations that may result in a fire, an explosion, a fatality or the release of a substance in a reportable quantity. Mitigation plans and a bypass permit must be approved before a critical safety device can be disabled. Examples Pressure relief valves Emergency stop cables and buttons Fire protection systems Process safety interlocks Machine interlocks on rotating equipment Air monitors tied into a system (i.e. oxygen, CO, LEL)

Reporting and Investigation
Mosaic is fully committed to protecting your life and these Life-Saving Rules are vital to this commitment. Workers are empowered to stop all unsafe acts and are accountable for the safety of themselves and their colleagues. Because not following these or other safety rules could put you and others in danger, near misses and violations are expected to be reported so we can learn from them to prevent future incidents. We’re all responsible for safety. Violations will result in an investigation and could result in disciplinary action – not just for those who violated the rule, but for others who allowed it to occur.
