Standard Agency Termination Process Overview

PI/Other Business/Legal/Agency/Other initiates the termination action
Internal Communications occur, including copying and/or sending s to Ryan Hogan (PI Sales Operations)
Beverly Gilman is Ryan Hogan’s back-up
Ryan assists Field with compliance with process (answers questions, confirms steps, etc)
PI initiated termination, PI Field will send request for termination letter to Ryan Hogan/Beverly Gilman, Sales Operations
Blue = PI Sales/Finance/Ops –R. Hogan
Teal – Agency Administration- S. Hurley
Yellow - Field Support Desk
Green – Fields Support Desk & PI Sales
Ryan will forward request for a termination letter to the Field Support Desk
at the following address:
Draft of Termination letter will be sent to individual, whose letterhead will be used, for his/her review and approval with a copy to Ryan Hogan (for procedures related to the creation of the letter, please contact Field Management)
Individual, an AA , RSD or SS replies to Field Support Desk with changes or approval with copy to Ryan Hogan, PI Sales Operations
Once approved, Field Support Desk will send final letter with e-signature to Requestor/Signator, and copy of letter to Agency Administration (for procedures contact Field Support Desk or Agency Admin)
Letter can be mailed by the Field Support Desk or the letter can be sent to the Sales office for personal delivery.
Agency Administration will take the appropriate steps regarding the termination (see Agency Admin procedures, Sharon Hurley)
Field will send copy of termination letter to Ryan Hogan and a Sales Organization representative with knowledge of the termination will take the appropriate steps to complete the termination, and will send an confirming the termination to appropriate parties
