1. Course Numbers (All capitalized)
a. Ex: MSC 011
2. Course Titles (Abbreviated or full title)
a. Ex: (ABBREVIATED) Intro to Army
b. Ex: (FULL) Introduction to Army
3. Hours and Credits (same amount)
4. Grades (put exact letter grade and sign from unofficial transcripts)

4. Filling Out Your Plan Major Courses:
Use the outlined semester progression for your major to fill out your 104-R.
MSC Courses:
Add in your MSC course progression.
Military History Course:
Select one compliant history (these are listed under 104-R resources on our website).
Full Time Student:
You must have at least 12 units each Fall and Spring semester. Your MSC Course(s) count towards that
total.

5. Submission Process
Once all the requirements have been filled out on your academic plan, send the excel version for review.
DO NOT SEND A PDF VERSION.

6. Revisions
REVISIONS NEEDED: If revisions are needed, I will send a response with corrections. Please make the corrections and send the corrected 104-R back to me.
NO REVISIONS NEEDED:
● CC’ed on to Graduation Advisor for ROTC
● Check your !
● The certified 104-R will be sent to my office.
