1 Using and Navigating the AACVPR Program Certification Annual Report Platform
AACVPR is pleased to offer a new process of submitting an annual report for program certification. This brief presentation will guide you through the steps of this new process as well as its inner workings. It is not intended to be a source of information for clinical questions, it is purely a technical guide for navigating the system. If you have questions on what the annual report is about and why we are implementing this, please refer to the AACVPR website. Click the audio button on every page to follow along with the presentation.

2 Accessing the Annual Report Dashboard
All AACVPR pulmonary or cardiac rehabilitation annual reports will be entered through the online process only. The annual report will be open from May 7 with the submission deadline of July 31st

3 Accessing the Annual Report Dashboard
You will access the AACVPR program certification application through the AACVPR website at Once logged in and on the home page, select the My Programs button This annual report for program certification is accessible to both AACVPR members and non-members who are preparing to re-certifying for program certification. For non-members, you will still need a profile with AACVPR to submit an application (circled on screen shot)

4 Accessing the Annual Report Dashboard
After logging in and selecting the My Programs button, for organizations with both cardiac and pulmonary programs, you will see both programs listed as we do on my screen. For organizations with only cardiac or only pulmonary, you’ll be able to click the Program Certification Application button next to your facility and be brought directly inside the annual report.

5 Navigating the Annual Report
Now that I’ve clicked to open my facility’s application we are now brought into my dashboard. I can see an option to start my Cardiac program’s annual report. Click the button to begin.

6 Navigating the Annual Report
The annual report is grouped into 9 pages, each annual report page represents a page on the actual program certification application. The requirements drop down we see in the attached image represents your progress through the annual report. All 9 pages are required. As you complete each page, the drop down will reflect your progress. You will notice that sections and buttons are color coded, and the text on the button indicates the next action needed in the process. The color of the button tells you the status of that step. Gray buttons mean the page referenced still needs to be completed by you. Green buttons indicate that you have completed the page and no further action is required.

7 Navigating the Annual Report
Once you scroll past the Requirements drop down, we get a deeper look into the annual report. In this image we can see each page and the questions that will be asked for that page. Click on any of the “Page” buttons to begin.

8 Navigating the Annual Report
Clicking on the “Page” button will pull up a pop up. Included on this first pop up is a full review of this pages requirements. For example, I am reviewing the Program staff and competencies page of the application. Included in this description is the overview, tips, reasons for denial, and required elements all for the program staff and competencies page of the application. This is here for your review in preparation your upcoming recertification. Be sure to review everything to make sure you’re on the right track to meet all requirements for recertification. When you are finished reviewing the page, click “Next”.

9 Navigating the Annual Report
Now that we’ve completed the first step, you’ll notice that the button has switched from “page” to now read “annual report”. Since the button is still gray, that means there are more requirements before I can move on the next section. Click on the gray button labeled “annual report”. A new pop up appears and lists a question. Select your response with the drop down and click “Next”

10 Navigating the Annual Report
The same behavior occurs in which the gray annual report button has now turned to a gray “Reviewing guidance” button. We still have more requirements before we move on. Click on the gray “reviewing guidance” button. Review the next pop up, and click “done” when finished. After this, the sections button will turn green and read “Completed” indicating that this section is complete. Repeat these steps will all other pages of the annual report.

11 Completing the Annual Report
Once you move through and complete more pages of the annual report, you’ll start to see the requirements drop down indicate your progress. If the progress bar or buttons on each page do not turn green, that means you have not met all of the requirements for that page. As I can see if the image provided, I have completed 3 pages of the annual report and I have 6 pages left to complete

12 Completing the Annual Report
When all buttons are green and the requirements tab indicates that you are complete, you can click the “Pending” button at the bottom and complete the final steps. The final steps include verifying demographics and completing the attestation. Do not that all demographics can be updated in your AACVPR program profile. Once everything is correct, indicate your response on the attestation and click “Submit”. If you see a green submit on the bottom of the page, you have successfully completed the annual report and no other actions are required of you.
