7 2018 Safety Performance Lost Time Injury Frequency (LTIF) achieved a new record of Total Recordable Case Frequency (TRCF) achieved a new record Results are overshadowed by the tragic two work-related fatalities. Zero road work related fatality since 2016. Road Safety 800 Million km driven without road traffic accident fatality. - Define LTIF what does it mean? - Define TRCF what does it mean? 2019 LTIF target 0.15 ? 2019 TRCF target 0.71 ? Definitions: LTIF (Lost Time Injury Frequency) The Lost Time Injury Frequency is the number of Lost Time Injuries per million man-hours worked during the period TRCF (Total Reportable Case Frequency) The Total Reportable Case Frequency is the number of Total Reportable Cases per million Exposure Hours worked during the period More definitions: Total Recordable Cases (TRC)- Total Recordable Cases are the sum of Fatalities, Permanent Total Disabilities, Permanent Partial Disabilities, Lost Workday Cases, Restricted Work Cases and Medical Treatment Cases. Sometimes referred to as Total Recordable Cases. Lost Time Injury (LTI) are the sum of Fatalities, Permanent Total Disabilities, and Lost Workday Cases. N.B. If, in a single Incident 20 people receive lost time injuries, then it is accounted for corporate reporting purposes as 20 LTI's (not 1 LTI). Fatality (FAT)- A fatality is a classification of a death resulting from a Work Injury, or Occupational Illness, regardless of the time intervening between injury/illness and death. Note: the number quoted in this pyramid does not include third party fatalities. Unsafe Act - Is an action by a person which could have led to an injury, damage or harm, but did not result in any on this occasion. Unsafe Condition- Is a condition of a worksite which could have led to an injury, damage or harm, but which did not result in any on this occasion Near Miss - It is an unplanned event that did not result in injury, illness, or damage to assets, the environment or Company reputation – but had the potential to do so if some circumstance of the event were different. Incident With Consequence - an unplanned and undesired event or chain of events that has resulted in injury or illness, damage to assets, the environment, company reputation, and/or consequential business loss. AIPS Incident/ Near Miss - A process safety event is an incident that resulted in, or could potentially have resulted in an unplanned or uncontrolled release of: Combustible liquids (e.g. MEG, TEG, diesel, lube oil, hydraulic oil, etc.); Flammable liquids (e.g. crude oil, methanol, IPA, etc.); Flammable gas (e.g. natural gas, butane, pentane, etc.); or Toxic chemicals (e.g. H2S, SO2, mercury, etc.); or Non-toxic and non-flammable materials (e.g. steam, nitrogen, compressed CO2 or compressed air) that result in actual consequences. 9 The Basics of an Observation What is Behavior Based Safety?
Behavior Based Safety (BBS) commonly is known as a process that is intended to focus workers attention on their own and their peers daily safety behavior. When implementing a BBS program, observers (employees who are trained) conduct observations of other employees with an eye on their behavior. This is done through an observation process. The goal of a BBS program is to improve the employee safety in the workplace. These observers record safe and unsafe behaviors, in addition to noting safe and unsafe workplace conditions. The observer then shares the findings with the worker and provides feedback. BBS is a broad term used to describe everything from basic everyday employee behaviors, to more comprehensive strategies to change a company's safety culture. BBS has been around since the 1980’s, and evolved over time with new findings in behavioral science BBS programs are based on a continuous feedback loop where employees and observers provide input on improving safety to each other and safety professionals utilize the data collected in conducting the observations to continuously improve safety. Positive feedback is encouraged. Discussing the ways in which employees can perform their tasks in a safer manner helps workers and observers to become more aware of their behavior.    9

10 The BBS Observation Card
Organizations who implement a BBS program determine the appropriate list of behaviors to observe based on the unique behaviors and risks of their organization. Safety professionals usually develop a checklist format that is easy and quick for observers to complete in the field and lists the target behaviors.  Checklists often include the basics of the observation (time, date, location, behaviors observed, observer) and the number of safe and unsafe observations of the reviewer.  The checklist also includes fields for any feedback and comments provided to the employee.

14 WHY? Available Facilities
Many staff are still using their private cars, In this slide you show first the company provided transport facilities' (Air & Land), these means of transport are very safe and we didn’t have a single fatality recorded up to date compared with private commuting which was 89 since 2003. Although we do have these facilities', yet many staff are still using their own private cars for commuting….now you need to ask the biggest question….Why? Listen to their views and reasons highlighted by them and make a note of them. WHY?

15 Why is it an issue? Total of 89 Fatalities
PDO is very concerned about private commuting and it is seen to be a very serious issue within the organization… on the other side employees either PDO or contractors doesn't seem that they see it as an issue and that’s why we need to help them to change their mind set towards this issue. Before showing the statistics just through this question (Why the private commuting is an issue in PDO community?) give chance to the audience to share their views and opinions ….probably you will get different views and opinions …close the discussion by showing the statistics which is a sold proof that yes it is a serious issue. Close this slide and keep everyone start thinking about it while you start injecting the following slides pictures.

The selection criteria is simple ; what has the individual done to contribute towards enhancing the safety culture in the organisation. Awards should be limited to certificates. 17

18 Safety Day Actions At the end of the session: Suggestions/ Ideas to eliminate private commuting in the interior within your teams sent to corporate HSE. Individual Commitment and Ownership of actions taken by your team to stop private commuting. Take photos of your session and upload via the Safety Day Page - contractors can send their photos through their respective Contract Holder (CHs) to upload.
