Procedural Overview Agent uses case selection grid to select groups
Agent completes Prospective Employer Questionnaire and s or Fax's to the Travelers Sales Executive assigned to the agency. Travelers Sales Executive will review group characteristics and provide the agent with a response as to whether the group is acceptable. If the group is acceptable, the Sales Executive will contact the agent with the approval and request the signed Benefits Plus Marketing Agreement (PL-8602 or PL-9498). The Sales Executive will forward the Prospective Employer Questionnaire, a copy of the signed Benefits Plus Marketing Agreement and the form titled New Benefits Plus Request for Client Code, to the Benefits Plus Unit via to The Benefits Plus Unit will provide a Benefits Plus Code/Client ID, usually within two to three weeks. The agent can then begin to bind and issue policies. Payroll Deduction Authorization Forms (PL-8356) are to be completed at the time the insured completes an application for insurance. One copy of this form is to be sent to the employer as evidence of their authorization to make deductions. EFT Payment Plan Authorization Forms (PL-8355) are to be completed and a copy of a voided check attached and kept with the agent’s file for those policies to be billed as EFT.

Look to see if most are married and over 30 2. Look for average or better than average salaries for your area 3. Turnover under 15% is good anything over that should prompt more questions to figure out why. 4. Look for employers who are willing to help promote the program. The more yes answers the better.

Key reasons to sign the Marketing Agreement: Protects the agent-lists them as the agent of record Includes a hold harmless agreement Keeps Department of Insurance happy PERSONAL LINES – BENEFITS PLUS PAYROLL DEDUCTION MARKETING AGREEMENT This agreement is made and entered into this _______________ day of __________________, 2____ by, and between Travelers, and certain of its affiliate insurance companies engaged in writing business under the Benefits Plus Program defined herein (hereinafter called the “Company”) and _______________________________________________________________________ of _____________________________________________ (hereinafter called the “Sponsor”). 1. Duties and Obligations of the Company Company will offer a program (Benefits Plus) to eligible employees or members of Sponsor under which said employees or members, meeting Company underwriting requirements, may voluntarily purchase certain personal insurance coverages. An eligible employee is one who is designated as full-time permanent employee of the Sponsor. An eligible member is a current member in good standing of the Sponsor. B. Company will issue all coverage under the Benefits Plus Program through individual policies of insurance. The Company may refuse to write, cancel, change, or terminate the policy in accordance with the individual policy conditions and Company’s underwriting standards. Further, Company may refuse to write, cancel, change, or terminate policies in accordance with any Company withdrawal from writing any or all lines of personal property-casualty insurance in a state. C. Company will furnish to Sponsor and employees or members who have insurance under the Small Group Program notice of any material changes to the Benefits Plus Program. 2. Duties and Obligations of Sponsor A. Sponsor agrees to allow the agent of record for the Benefits Plus Program as specified in Section 2(B) of this Agreement (The “Agent”) to conduct the Benefits Plus Program promotional process which will include those activities indicated below: an announcement letter reproduced on Sponsor’s stationery and sent by Sponsor to all employees and/or members; a group meeting at the work site during working hours for the purpose of introducing the Benefits Plus Program: with mandatory attendance with voluntary attendance with management in attendance to introduce the Benefits Plus agent of record; posters to be placed on employee/member bulletin boards and/or in strategic areas; brochures and other Benefits Plus materials to be located in an area common to all employees/members; access to employee/members during working hours via telephone for the purpose of explaining the program, enrolling new participants, providing quotations, handling changes for participants, and the like; access to employees/members via periodic on-site visits; access to internal mail distribution facilities for the purpose of promoting the program; access to internal company publications for the purpose of promoting the program; access to a list of current employees/members; Sponsor will provide all new employees/members with Benefits Plus promotional materials as part of new employee/member package.

B. Sponsor agrees to promptly notify the Agent and Company’s Statement Billing Unit of all additions and/or deletions from Sponsor’s employee or member group. The Sponsor shall utilize its best efforts in promptly notifying the Company of any employee or member who terminates employment or membership with the Sponsor, or who does not, for any reason, have sufficient monies from which the proper amount for payment of premium can be deducted. The Sponsor shall not be held liable for failure to make such deduction, unless that failure is either reckless or willful. C. Sponsor will honor payroll or account deduction authorizations acceptable to Company that have been executed by eligible employees or members under the Benefits Plus Program. In the event employee or member of Sponsor authorized Sponsor to payroll or account deduct their premiums, Company will provide Sponsor with such billing statements on a monthly basis or on such basis as agreed to by the parties. Sponsor will remit all deducted premiums to Company within ___ days from the date Sponsor deducts the premium from its employee’s payroll or its member’s account. Sponsor assumes no responsibility under this agreement for the payment of any premium owed by an employee or member of Sponsor insured by the Company; provided, however, that all premium collected by Sponsor via payroll or account deduction is the property of Company and Sponsor is responsible for remitting the premium to Company and for holding such premium as a fiduciary until the premium is remitted. D. It is agreed that ________________________________________________ of ____________________________ will be the agent of record for the Benefits Plus Program subject to the terms of the Agent Agreement entered into by Agent of Record and Company in relation to the Benefits Plus Program. Sponsor agrees not to replace said Agent of Record with a new agent except for good cause. Sponsor will not replace said Agent of Record unless Company consents to such replacement, which consent will not be unreasonably withheld. Effective Date This Agreement will take effect on ____________________, 2____. 4. Termination Either the Sponsor or the Company may terminate this Agreement subject to ninety (90) days prior written notice to the other party, unless both Sponsor and Company mutually agree to a different period. 5. Eligibility As a condition of eligibility, each employee/member at the time of application for insurance shall authorize the Sponsor or Company to deduct from his/her payroll or account an amount sufficient to permit payment of the total premiums due the Company for such insurance. 6. Limitations Imposed on Company Company will use Sponsor provided employee or member lists or policyholder records only for the solicitation of personal lines insurance under the Benefits Plus Program. Unless specifically authorized by Sponsor in writing, Company will not use such records for any other purpose not agreed to by the parties, including the sale or sharing of records with third parties. Notwithstanding the foregoing, the parties understand and acknowledge that the use and control of the policy expirations and records related to business written under the Benefits Plus Program will reside in the undisputed possession and ownership of either the Agent of Record or Company, as provided for in the Agent Agreement entered into by Agent of Record and Company.

7. Limitations Imposed on Sponsor Sponsor is not an insurance agent and shall not act as an insurance agent under the Benefits Plus Program. 8. Indemnification Company agrees to indemnify and hold harmless Sponsor from and against any claims, damages, cost, and expenses, including reasonable attorney fees, arising out of any acts, errors, or omissions of Company in the performance of its obligations under this Agreement. 9. Amendments This Agreement may be amended, modified, or supplemented only by written agreement signed by each of the parties hereto. 10. Entire Agreement This Agreement constitutes the entire agreement between the parties relating to the matters contained herein and supersedes all previous communications, representations, or agreements, either oral or written, with respect to the subject matter hereof. IN WITNESS WHEREOF, the parties have executed this agreement as of the date first written above. Adding a New Database to Your Lotus Notes
There are three ways that you can add a database to your Lotus Notes workspace. The first and easiest way to add a database is to have somebody send you a link to it. When the link arrives in your , it should have the file extension “.ndl.” Before double-clicking on the link, you must first open up Lotus Notes, come back to the , and double-click on the link. Select open and it will automatically launch the database. When you close the database, the database icon will remain on your desktop. Training documentation and links to icons can also be found on the MRC/FSRC. All Lotus Notes materials can be found under the “Increase Productivity” section. Find the database you are looking for and you can add the link from here. This requires that you have an existing link to the MRC. If you know which server your database is located on, you can also add the database from Lotus Notes. Most PL databases are on the Notes 035 server, see the list below. From Lotus Notes go to File (upper left corner)-Database-Open.
From the drop-down box select the server where your database is located. In the “Database” box scroll down to the yellow folder labeled “PL” Double click on the PL folder, and find the database you are looking for. Highlight your database and select the “Open” button, which will launch the database. When the database is closed, the icon will remain on your Lotus Notes desktop. Creating a Database Link to Send Through Email
Open Lotus Notes and Right Click on the database you would like to send. Scroll down to “Database” - and scroll over to “Copy as Link” Go to the Start Menu and click on “Programs” - “Accessories”- “Notepad.”

Right click in the notepad and select “Paste”
This will paste a formula in the notepad. Go to File-save and save the notepad as the database name followed by the file extension “.ndl” Attach the file you have just created to an and send it off.
