Modifying forms Forms can be modified for jurisdiction- or discipline-specific needs Add the suffix “a” to the form name or block you add/change E.G. ICS 214a or Block 2a “a” is for Adapted Forms can have an appendix or additional page Continue with sequential block number

Helpful hints Instructions are included with every form
Remember when printing Turn in your forms to your supervisor prior to leaving or leave them at staging when you sign out of an event Electronic fillable forms are great But keep hard copies with you Keep notes or voice recordings to fill out forms later If possible, keep a copy of your completed form
