THGPCG Approach We will build a culture which is not based on blame – none of us can say we have never made a mistake! We encourage people to report events – both positive and negative We will to learn both from good practice and from safety issues - this can only happen if people record and report incidents We will work within our teams and with partners to implement solutions to prevent harm – remember most issues arise because of systems not people.

When should you report? As soon as possible after the event
Any incidents you have been involved in Any incidents you have witnessed Incidents that have caused no harm or minimal harm as well as those with more serious consequences Prevented safety incidents (near misses)

What you should report Clinical Issues: e.g. Medication, poor transfers of care, medical device failure, delays in care Environment Issues: e.g. Accidents, violence and aggression, staff health issues related directly to their work Professional Issues: e.g. Breaches of confidentiality, standards, conduct issues Service Issues: e.g. loss of data, loss of service
