4. When should you report? As soon as possible after the event
Any incidents you have been involved in
Any incidents you have witnessed
Incidents that have caused no harm or minimal harm as well as those with more serious consequences
Prevented safety incidents (near misses)

5. What you should report
Clinical Issues: e.g. Medication, poor transfers of care, medical device failure, delays in care
Environment Issues: e.g. Accidents, violence and aggression, staff health issues related directly to their work
Professional Issues: e.g. Breaches of confidentiality, standards, conduct issues
Service Issues: e.g. loss of data, loss of service
