Arrest/charge/conviction for OUI or other substance-involved criminal conduct

Violation of MPHP monitoring agreement and/or consent agreement

Remain substance free
Inform employers/education program of Consent Agreement
Monitoring Program (Medical Professionals Health Program)
Submit quarterly progress reports from treatment provider(s) & nurse employer(s)
