Choose Cross Country, Girls or Boys

6. Fill out all STUDENT information

7. Click to download physical form for doctor to fill out
If you have a digital copy of the physical you can upload the file, if not just click Save to continue.
Click Save

8. Answer all Medical History questions

9. Fill out Parent/Guardian Information

10. Parent must digitally sign all fields

11. Students must digitally sign all fields

12. Click Back To Clearances

13. Click on View

15. If you are planning to play other sports, even if you are just considering it, just click on the additional sports and hit submit.
