2 Figure 1 Kidney biopsy revealed 19 glomeruli with (A) mesangial expansion, fragmented erythrocytes (small arrows), and microaneurysms (white arrows), and some mesangial areas showed (B) rarefied matrix (“mesangiolysis,” arrows). There was also (B) endocapillary hypercellularity (asterisk) and mild tubular atrophy and interstitial fibrosis. Medium-sized arteries displayed intimal fibrosis that occluded 20% of the lumen. (C) Electron microscopy showed edematous endothelial cells with loss of fenestration and detachment from the glomerular basement membrane (letter e; original magnification, ×1.9) by the interposition of (C, D) an amorphous granular material (black stars).
