3 Forms for Programs: (Applies for Summer classes only)
ESL, Citizenship & Computer classes will need : Registration form Supplemental form Emergency Card GED classes will need: Program Entry/Update Testing form

4 Registration Form: Students should complete highlighted areas
Please review form to make sure all areas have been completed

5 Supplemental Form: Emergency Card
Students should complete form if applicable Answers from sections #’s 2 & 3 should be bubbled on Entry form under section #14 (Only GED classes) Emergency Card Students should complete highlighted areas Keep cards for your records Contact information should be the same in ASAP

6 Deadlines: Transfers/Drops:
Registration will be taken the first 2 days of class All registrations and additional forms should be submitted to our office the same day or next day at the latest Transfers/Drops: If any students attend your class and complete a registration form and then drops or transfers, please submit their registration and enter their attendance hours into ASAP. Once their attendance hours have been entered, you can drop them from your class Transfer students are required to complete a registration form for their new class We want to capture all student attendance hours even if they only attended 1 day

7 GED – Program Entry Form
Bubble Sheet – Important Items: Section # 3 – Students should complete bottom portion of Student Identification box Section # 6 – If students attended school outside of U.S., they should complete bottom portion of box Section # 7 -If students earned diploma or degree outside of U.S., they should complete bottom portion of box Section #11 - All students should mark “Basic Skills”(ABE)

8 GED Testing: Pre-test no later than June 4th
If students are not in database, you’ll have to paper test Post testing should be completed on or before June 30th

9 GED – Program Update Form:
Bubble Sheet – Important Items: Students should complete sections #’s 1- 6, & 8 -10 Section # 6 – Students who are no longer attending and left before attending 12 hours, mark “No show”. Students who attended 12 hours or more, mark “Left program”. Section # 9- Students who have progressed to a higher level should mark “Educational Achievement” Section # 9 - “Skills Progression” should ONLY be used for CEP programs.

10 Paperwork: Before you submit your paperwork:
Review paperwork for accuracy and missing information If missing information, please ask student to fill in information All paperwork should have section number and instructor name Inaccurate or missing information will delay processing your paperwork.
