Falling From Height – Broken Arm
HSE LEARNINGS INCIDENT: The mechanic was attending a breakdown to 50 Ton Crane. After filling the water in the supplementary tank connected to the radiator, he was coming down from the crane platform when he slipped. He lost his balance due to wet surface and his foot got entangled in the brackets provided for water flask. He fell from a height of 1.4 mtrs landing on his left forearm causing multiple fractures to it. Root Causes: Lack of due care and attention Inadequate supervision LEARNINGS: Dynamic risk assessment and subsequent control measures. Key Learning: Always pay attention to footing, while working at height.
