A Pipe Layer was lifting a 36” pipe string (length m) in order to elevate the whole string from ground level to 0,5 m above
A lifting sling was used
After the pipe is lifted by the pipe layer wood staples were set up under the string by two workers
The workers were located at each side of the pipe string

The lifting sling breaks, the pipe falls down on the wood stack just between the two workers

Cause of the Incident
Cause: The sling breaks because the weight of the pipe string was too heavy for the sling (lifting capacity 8t) Root Cause I False assumption of the lifted weight by the responsible foreman: Distance between the support of the wood stack and the lifting of the pipe: less then 18m Weight of one 18m pipe: 6t Weight to be lifted less than 6t

Cause of the Incident
Root Cause I: Fact: The lifted distance of the pipe string is much higher than the assumed 18 m : By lifting a pipe string of this dimension the real lifted distance almost the double way and the lifted weight is about 18t

The foreman did not got any instructions for these works:
No situational hazard analysis
No formal standard procedure
No briefing

Measures taken from this incident:
Design calculation of all possible lifted pipe strings from pipe diameter 4“ to 56”
Preparation of a method statement for lifting pipe strings with exact information of the capacity of the slings to be used.
