Provide color photographs and a map depicting the proposed activity areas. Provide a cross section of the proposed culvert depicting the existing and proposed conditions, stream bed, normal water surface, and culverts. Provide plans and details for the proposed culvert depicting the size, opening, footers, bottom. Revise the arch culvert design to include construction of a normal and low flow channel through the structure which does not over­widen the stream channel. Provide a wetland determination and delineation report with data sheets.

Provide a project description which identifies and describes the purpose of the proposed channel changes upstream and downstream, how these changes affect the overall stream channel stability and slope, and why this is appropriate and stable for the stream channel. Identify and provide specific details on the material placed in the downstream channel elevation increase to raise the stream bed. Provide measures to ensure that the stream bed downstream of the culvert does not scour and erode. Other elements???
