2. Background to development
Part of Good Life for All project - working together with Providers across central to improve people’s quality of life
Worked with colleagues from PCT & 2 Hospitals to develop forms that will be recognised by hospital staff
Aid to improve communication between people, provider staff & hospital staff .

3. Why have hospital passports?
Hospital Passports have been developed nationally to support adults with a Learning Disability - who have sometimes received poor care and support in Hospital - positive outcomes
Used to share important information about people’s needs
Improve the experiences of people when moving from one environment to another
Promote a joined up approach in supporting individual’s care
- Support vulnerable adults especially for those who have limitations regarding their communication skills and their capacity

4. What are hospital passports?
- Used to document essential information about an individuals health and wellbeing
Can be completed by the individual or with support from family member / support staff - useful aid to promote communication
Its purpose is to let medical staff know necessary information about the patient to allow for more effective treatment
More importantly it helps to reduce confusion and uncertainty for the patient whilst in unfamiliar surroundings
Allows hospital staff to communicate in the best way with the patient
- Doesn’t replace the detailed information in a patient file or in a care or support plan - but compliments it

5. What does it look like? -Usually colour coded – red, amber green.
-Ideally should not be more than two pages long.
- The suggested version has been adapted from other passports e.g. LD.

7. Progress so far.....
- Developed a hospital/health passport as a benchmark to illustrate how people are when they are well
- In addition to this, worked with health colleagues to develop a Care Home Transfer form
Shared documents with some Providers (res and nursing care homes) positive feedback
Held 3 sessions with Provider staff - feedback is positive
Sharing with yourselves at the Provider Forum (29/09/11)
Providers will be asked to start using the forms
Review will take place March 2012
