According to CDC statistics, 1 in 2 adults suffers from a chronic condition such as heart disease, lung disease, HTN, DM, arthritis. 1 in 4 adults suffer 2 or more. Modifiable risk behaviors such as lack of exercise, poor nutrition, tobacco and alcohol abuse account for much of illness and early death leading to chronic disease. The cost of chronic illness was over $2 trillion in 2014

4 25% Prevalence of Diabetes in Seniors: The percentage of Americans age 65 and older remains high, at 25.2%, or 12.0 million seniors (diagnosed and undiagnosed). American Diabetes Association.

5 53% Chronic pain in later life is a worldwide problem. In one nationwide survey of older adults (n=7601) in the United States, 52.8% reported experiencing bothersome pain in the preceding month.10

6 $50B 1 in 3 adults over the age of 65 fall each year. Falls are the leading cause of fatal injury among older adults and half of non-injured fallers cannot get up without assistance. Fear of falling is a recognized syndrome among older adults and causes adults to limit their activities leading to further physical decline, depression, social isolation, and feelings of helplessness. In 2015, there were over 2.8 million injuries treated in EDs due to falls, over 800,000 hospitalizations, with a total cost of over $50 billion.

The Juniper network helps to achieve those goals. Builds on an existing network AAAs – this loosely connected network had been trying to spread evidence-based health promotion for years with little success. In the Juniper network, AAAs act as a local hub, building relationships, managing resources, and connecting to other branches of the network. Each of the AAAs develop relationships with community-based organizations (community centers, housing centers, faith-based organizations) and healthcare organizations to offer Juniper classes. We train and certify community members as class leaders – these are lay leaders, peers to those who participate in the class.
