Questions to Staff Do you have any safety concerns you would like to share? Answers below Concerns Action Plan Responsible Party 1 Standard questions for staff Can you think of an event in the last few days that resulted in prolonged Hospitalization? Have there been any incidents lately that you can think of where a patient was harmed? Have there been any near misses that almost caused patient harm but didn't? What aspects of the environment are likely to lead to the next patient harm? Is there anything we could do to prevent the next adverse event? Are we actively promoting a blame-free culture? Can you think of a way in which the system or your environment fails you on a consistent basis? What specific intervention from leadership would make the work you do safer for patients? What would make the safety Walk Rounds more effective? What AWH administration can do to increase your satisfaction as an employee? What could affect your job satisfaction? We’re going to work on the information you’ve given us. In return, we would like you to tell two other people you work with about the concepts we’ve discussed in this conversation

28/05/2014 OPD Staff not aware about routine procedures e.g. nasal swabs for MRSA Feah (Infection Control) We had the MRSA session with the staff – we are also are in the process of following up the MRSA rescreening patient flow. We will be finalizing the patient MRSA rescreening flow with the committee and with Dr. Ezeldin as well when he comes back from leave. The IPC meeting is on Sunday, so we will present the suggested patient flow Equipment sterilization and usage not appropriate in ENT clinic. OPD Team Issue discussed with CSSD Supervisor and Head of ENT Dr. A. Jabbar. 7/5/2014 ER Improper patient identification by staff Alaa Adult ER Head Nurses were informed to arrange education for ER staff 10/6/2014 Pharmacy Pharmacy labels (with drug dosing) should be changed to images instead of instructions in English, which will make it easier for patients to understand. Rasha Dr Wesam: pictograms project has been established in the See & Treat Pharmacy since September
