Name of the Intervention: Client enrollement assessment and expectations.
Primary Driver: 1) Retention/HV completion. 2) Establish trusting partnership btw/ home visitor and client.

2. Definition of the Intervention
What is the intervention? Include a short definition of the intervention (1-2 sentences)
NHV team will obtain 1,2, and 3 contact telephone numbers and names from client at enrollment visit (example: client, father of baby, client mother, trusted friend, etc).
NHV team will document names and numbers on enrollment form and obtain consent forms with signtatures.
NHV team will create new talking points to ensure families understand the scope and length of service at enrollment visit.
NHV team will document scope of service conversation and teach back in narrative nursing note.

3. Description of the Intervention
Include photos, and instruments or tools that accompany the intervention (include tools that are for doing the intervention itself, like the Marion half- sheet for communicating results to parents, and tools that are used for collecting data on the intervention during PDSA cycles)
Enrollment forms, consent forms and narrative nursing note.

4. How is the intervention carried out?
Include instructions about how to do the intervention (as if you were explaining it to some one who has never done it before)
NHV team will begin obtaining 1,2 and 3 contact names and numbers at enrollment.
Team and supervisor will reevaluate data collection and retention in 1 month.

Include run charts and summaries of PDSA cycles (PDSA ramp of several cycles ideally)
Include qualitative information as well – such as testimonies from LIAs or home visitors or families who have used the intervention
How many LIAs / home visitors / famlies used the intervention ? Who are the experts who can provide more information?
