1. ADVANCING THE PRACTICE OF CASE MANAGEMENT
The practice of case management is a refined model of care coordination established more than a century ago (Joo, 2018). Case Management has been adopted into the American healthcare system but very little research exists that effectively demonstrates the strengths of a case management delivery model. The Agency for Healthcare Research and Quality identifies that 1% of high-risk patients account for 22% of all healthcare expenditures (Joo, 2018). The episodic and fragmented structure of the American healthcare system is responsible for these costs as well as poor quality care.
Case management is an effective, efficient, and cost-effective strategy to deliver patient- and family-centered care. Case management is tied to care coordination in the literature, but consistency in definition and scope are lacking. CMSA was the first organization to define case management and establish Standards of Practice, but these are not universally adopted. For case management, and CMSA to be recognized as THE strategy for improved care coordination and patient outcomes, CMSA must develop a theoretical framework to guide research that will demonstrate these improvements and clarify to the healthcare industry who a case manager is, and the role the case manager plays in the care of patients.

7. Case Management Research Need for Comparative Effectiveness
Need for research to support CM interventions that reduce fragmented care, improve quality of care, and impact cost through care coordination.
CMSA definition of CM and Standards of Practice are not universally adopted.
CM does not have its own theoretical framework.
Need for the development of tools that can measure CM effectiveness.
Most research in healthcare is conducted to compare the effectiveness and safety of a treatment for a particular disease or condition, but are not always designed to compare findings of two or more alternative approaches (Moreo, 2013). For example, Random Control Trials (RCT) are the most common framework used to compare a treatment to a placebo, but these results often cannot provide recommended alternatives for an individual or for a population (Moreo, 2013). Comparative Effectiveness Research (CER) is a framework that examines evidence for individual recommendations or for those of a population (Moreo, 2013). Based on examples of CER on the treatment of chronic illness, the potential for application to case management was explored.
A study in 2013 using a CER framework revealed case management improved outcomes, quality of life, and access to condition-changing care (Moreo, 2013). To improve case management research, case managers are encouraged to learn more about CER, is implications for development of tools and resources that result in the delivery of the best possible care for the individual.
Another missing piece according to Moreo, was the lack of inclusion of CMSA’s Standards of Practice to CER.

Development of processes that will result in improvements in:

These are examples of Process Improvement projects that individuals or organizations can undertake to demonstrate the value of case management. Care that informs and involves patients in medical decision making and self-management.
Coordinates and integrates medical and psychiatric care.
Provides physical comfort and emotional support.
Understands the patient’s concept of illness and their cultural beliefs.

Understands and applies principles of disease prevention and behavioral change appropriate to diverse populations.
Acknowledged as an effective and efficient approach to care

Case Management is a complex process. Case managers need to maintain competence in working with individual patients across the continuum of care as well as managing patients within a population health model.

11. CMSA Foundation Scholarship and Grant Opportunities
A primary Foundation endeavor is to support the development of educational resources and raise awareness regarding health care policy and how it affects the role of the professional case manager.

12. Margaret Leonard Public Policy Grant
A new grant funding opportunity supporting public policy efforts.
Government plays a major role in population health.
Laws, regulations and policy influence how professional case managers practice within their role.
Funding for this grant is made possible by Margaret Leonard, CMSA Past President and CMSA Foundation Past-President, who is dedicated to not only supporting the CMSA Foundation, but the promotion of public policy awareness.
This grant supports the public policy efforts of an individual striving to build a program that effects case management. This grant is made possible by Margaret Leonard, and her unwaivering support and dedication, plus her love for public policy.

13. Margaret Leonard Public Policy Grant
The grant will be awarded to an individual case manager who has a passion for case management, advocacy and helping to change healthcare policy by effectively communicating with legislators, regulators at the local, state, or national level.
The intent of this grant program is to assist an individual in creating a method to effectively promote and raise awareness regarding the Case Management Model Act, Standards of Practice for Case Managers, Transitions of Care, Care Coordination and other public policy topics supported by CMSA.

14. Integrated Case Management Training Program Scholarship
A scholarship to attend CMSA’s Integrated Case Management Training Program.
This scholarship is offered to a case manager committed to improving health outcomes of the most at-risk populations and is willing to link and integrate care coordination across all domains of health.
Need support in completing the Integrated Case Management training? Applications for the 2020 training be due in January Training consists of an online, self-study component, and then a 2-day face-to-face session in June 2020 during the CMSA Annual Conference.
