Chronic Pseudoaneurysm Routine post-operative computed tomography shows an incidental finding of a chronic pseudoaneurysm below the left coronary artery (yellow star) with dynamic obstruction of the coronary flow. The site of rupture is not clearly identifiable. During TAVR, the patient underwent redilation for paravalvular regurgitation and implantation of a second transcatheter valve for subsequent transvalvular insufficiency. Note “valve-in-valve” (red bracket). The patient had no symptoms and refused surgery. She died suddenly 6 months after TAVR.
