Pemetrexed (Pmx) nephrotoxicity may result from proximal tubular cell uptake of drug through apical (folate receptor-α [FR-α]) and basolateral (reduced folate carrier [RFC]) transporters. After it is inside the cell, Pmx is polyglutamylated, which inhibits drug transport out of the cell and enhances drug inhibition of folate metabolism.
