1 MTPD Technology Overview
GetCare TCARE Welcome to MTPD Technology Overview. This is one in a series of courses designed to help the learner get a high level over view of technology required for the MTPD Initiative. This course was designed for Snohomish County Long Term Care and Aging MAC & TSOA case management contractors. BarCode ACES

2 TCARE Screening This course looks at the T-Care system. Specificly at the screening tool.

3 TCARE Screening Linking the Care Receiver to the Caregiver and to CARE
Creating the Screen Inputting the Screen Responses Completing the Screen Case Notes Lets look at the learning objectives for this course. Linking the Care receiver to the caregiver to the care systems. We will be looking specifically at the screening tool. With in the screening the tool. With in the screening tool we will look at how to create the screen, how to input the screen responses, and than how to complete the screen. Finally we will look at the case notes.

4 Linking Care Receiver to Caregiver and to CARE
Attach (link) a new Care Receiver to the Caregiver – Select the “+” button located above the Care Receiver list. This will initiate the Care Receiver Search screen. Find an existing Care Receiver or create a new one. The Care Receiver Search function will provide the option to Attach the Care Receiver. The Attach function is the link between the Care Receiver. Lets starts by Linking the care receiver to the care giver and then linking them both to care. You will need to link the caregiver receiver to the caregiver. To do this you want to click on the plus button located on the care receiver list this will initiate the care receiver search screen find an existing care receiver or create and new one. The care receiver search option will provide the option to attach the care receiver. The attach function is the link between the care receiver and the caregiver. Now if you need to remove the link between the care receiver and the caregiver select the care receiver than click on the minus button. Remove the link between a Care Receiver and the Caregiver – Select the Care Receiver then click on the “-” button.

Create TCARE/CARE Link: Click the Create TCARE/CARE Link Linking the care receiver and the caregiver to care click create TCARE link button.

Create TCARE/CARE Link: Click the Create TCARE/CARE Link has been clicked-- The following overlay will emerge—if the information is correct, click on the Finalize TCARE/CARE Link The following over lay will emerge. If the information on the over lay is correct click on finalize TCARE /CARE link.

7 Linking Care Receiver to Caregiver and to CARE
Once the TCARE/CARE Link is finalized, the Link can be viewed by clicking on the View TCARE/CARE Link Once the TCARE/CARE link is finalized, the link can be viewed by clicking on the view TCARE/CARE link.

8 Linking Care Receiver to Caregiver and to CARE
After clicking on the View TCARE/CARE Link, this overlay emerges with the HCS Financial Office of the client and the HCS Financial worker’s address After clicking on the view TCARE/CARE link, this overlay emerges with the Home and Community Services financial office of the client and the Home and Community Services worker’s address. This is helpful in formation for the MAC & TSOA case mananger.

9 Screening—Creating the Screen
First perform a Caregiver search, highlight the Caregiver and select Edit in Tree. Ensure the Caregiver’s name appears in the Tree to the left of the screen. Go to Action menu and select Create Screening. Now lets look at creating the screen. Frist we want to perform a caregiver search, once the name comes up in the search you want to highlight the name and than edit in the tree. Ensure the caregiver’s name appears in the tree to the left of the screen. Go to the action menu and select create screening. The pending screening will be displayed in the tree and available for completion. The Pending Screening will be displayed in the Tree and available for completion.

Notice the first six categories in the Assessment are actually the Screening categories. It is worthy of noting as a side by side comparison, when you put the TCARE screen next to the TCARE assessment that the first six categories in the assessment are the actual screening categories.

11 Screening—Inputting the Screen Responses
Click on the “+” the expand the Pending Screening. The yellow triangles mean the screen is not finished. As you complete each screen, the yellow triangle will turn into a green diamond. Next you want to click on the plus button to left of the pending screening yellow triangle. The yellow triangles mean that the screen is not finished. As you complete each screen, the yellow triangle will turn into a green diamond.

12 Screening—Completing the Screen
All fields with * asterisks are required. Select Action and click on Complete Screening/Assessment When you have completed all the screen responses, the triangles will change to green. Select Pending Screening, it will highlight. Remember all fields in the screen with an * asterisks are required. When you have completed all the screen responses, the triangles will change to green. Select pending screening when you click on it pending screening with highlight.

13 Screening—Completing the Screen
When the screen has been inputted and all of the triangles are green, click on the Action header and select Complete Screening/Assessment When the screen has been inputted and all the triangles are green, click on the action header and select Complete Screening/Assessment. When you do so the green triangle will turn to gray buttons and you will know that the screen in now complete.

14 Case Notes Case Notes – View or record information related to contacts with the Dyad or Providers to document activities. To search case notes by Contact Date Range, Last Modified Date Range, Contact Type, Purpose and Subject. Now that we have completed the screening lets move to Case Notes. In Case Notes section you can view or record information related to contacts with Dyads or providers to document activities. To search case notes by contact date range, last modified date range, contact type, purpose and subject. Subjects are useful when searching case notes. Subjects are useful when searching Case Notes.

15 Case Notes Case Notes – Add New Case Note – Select Add New Case Note, complete Contact Date, Submitted Date, Contact Type and Purpose. The subject line is a free text line along with the body of the case note. To add a new case note you will select case note button. Complete the contact date, type and purpose. The subject line is a free text area and should contain the body of your case note. When you have completed the case note click submit at the bottom of the screen to save the case note. *(This should state Put a brief description of case note in the subject line and the case note in the section under the Case Note heading) Click submit at the bottom of the screen to save the Case Note.

16 Case Notes Case notes can be amended. Add text using the Amend button. Strike out text using the Amend w/Strikeout button. ONLY THE ORIGINAL AUTHOR CAN AMEND A CASE NOTE. Case notes can be amended. Add text using the amend button. Strike out test using the Amend w/strikeout button. Only the original author can amend a case note.

First we looked at linking the care receiver to the caregiver that is in the TCARE system the two need to be linked. Then we looked at the screening tool how to create the screen how to input in the screen responses, completing the screen and we looked at creating and finalizing case notes.
