Family Guide: Understanding Transition Team Members’ Responsibilities
This resource was originally developed in 2012 by the Wisconsin Department of Public Instruction, Division of Vocational Rehabilitation and Department of Health Services.

Purpose This document is intended to help students with disabilities and families: Navigate the transition process Identify participants in the transition planning process Identify participants’ responsibilities Identify expectations of others based on your role in the IEP team Identify possible resources

Considerations Expectations are broad expectations.
Your local school district may have additional expectations Check in with your child’s school for additional expectations Feel free to add local expectations within the notes section

Acronyms to Consider: IEP = individualized education program
IPE = individualized plan for employment ISP = individualized service plan PCP = person-centered plans VR = vocational rehabilitation DHS = Department of Human Services SoP = Summary of Performance DD = developmental disability
