Comprehensive Primary Care Plus (CPC+)
CPC+ focuses on 5 key functions as related to primary care which includes
-Access and continuity
-Care management
-Comprehensiveness and coordination
-Catient and caregiver engagement
-Planned care and population health.
CPC+ is divided by two tracks.
Track 1 – Lower PBPM Care Management fees and incentive metric based payments, 100% FFS Office Visits, HIT Partners not required
Track 2 – Higher PBPM Care Management fees and incentive metric based payments, 100% FFS Non-E/M office visits, hybrid up front & lower FFS for E/M office visits, HIT Partners are required
This model includes 3 payment elements: Care management fee, Performance management fee and payment under the Medicare physician fee schedule.
