3. What is the HYS E-Survey Pilot?
The Healthy Youth Survey (HYS) is Washington State’s school-based youth health behavior survey.
HYS is a paper and pencil survey given every 2 years to students in Grades 6, 8, 10, and 12 (and Grades 7, 9, and 11 in small school districts).
Schools requested that HYS be available online and your school volunteered to participate in the pilot and will:
Receive 2019 survey results.
Provide critical feedback on the E-Survey process so it can be offered statewide in 2020.
HYS is sponsored by OSPI, Health Care Authority, Department of Health, and Liquor and Cannabis Board.

4. Who Participates in the E-Survey?
ALL 6th, 8th, 10th and 12th grade students in the school
Schools from eligible “small” school districts can also survey 7th, 9th and 11th graders
Participation is VOLUNTARY
Parents can decline participation
Students can decline participation in the survey (an alternative activity will be scheduled by your survey coordinator)
Students can skip any questions they don’t want to answer

You will receive the following materials from your school’s Survey Coordinator:

6. E-Survey Types Green online survey Blue online survey
For 6th graders (and 7th graders in “small” district schools)
Blue online survey
For 8th, 10th, and 12th graders (and 9th and 11th grades in “small” district schools)
There are two versions of the blue online surveys. They are randomized so half of the students will get Form A and the other half will get Form B.

7. E-Survey in Spanish
Students will have an option at the beginning of the E-Survey to select an English or Spanish survey

8. On the Day of the E-Survey
Plan to allow students a full class period for the online survey (about minutes)
Read through the Survey Administration Instructions
Follow your schools' protocol for setting up student computer access and entering the URL:
Enter the URL for all students, or
Bookmark the URL for students, or
Write the URL down so students can enter it themselves
Make sure you have enough Resource Lists for all students

9. Administering the E-Survey
Read the student instructions provided in the Survey Administration Instructions
Let students know:
Their answers are important!
Participation is voluntary (dismiss students to alternative activity if necessary)
The online survey does not include any codes or personal identifiers
No one will know their answers
The survey is not a test, there are no right or wrong answers
Ask students if they have questions before starting

10. During the E-Survey
Stay in the room, but do not wander around. Do observe students’ behavior to prevent students from finding out how other students answered and to prevent disruptions.
Answer students’ questions if they ask and you can preserve the anonymous nature of the survey. Let them know they can skip questions.
Accommodate students with special needs if reasonable, and if their answers can remain voluntary and anonymous.
If students finish early, ask them to sit quietly or read

11. Feedback on the E-Survey
Fill out the Survey Administrator Feedback Survey. A link will be provided.

12. When the E-Survey is Over
Remind students to go to the end of the online survey and Submit their responses and close their Internet browser

13. E-Survey Results
School building and district reports of results will be released in March 2020
The reports will include the same information as your 2018 HYS results
