6 Pharmacists prescribing via clinics, undertaking polypharmacy
reviews and signing prescriptions.
Establishment of link workers across the Locality.
Success in obtaining funding for 1 link worker to be based in a
voluntary organisation to focus on social isolation.
Trial of an open access mental health service.
Engagement with care at home providers.
Weekly meetings established from 18th March 2019.
Significant improvement in securing support for those in hospital or at
home.
