Required Paperwork Must have completed all Cabarrus County forms in order to be eligible to participate for a team. Athletic Participation Packet: Athletic Participation Physical Evaluation - signed and dated by a registered physician History form - signed by parent/guardian and student athlete - must be updated each year Domicile - on front of the participation packet Insurance Wavier form - if you need school insurance- kk.com- (It is also on our athletic webpage.) Gfeller/Waller Concussion Document - must be filled out by athlete and parent All MPMS athletic forms and packets can be downloaded from MPMS Athletics webpage.

Additional Information
All Athletes must dress out and participate in PE to be able to participate in their sport. Athletes will not be allowed to participate in any try out or open gyms until all required paperwork is turned in and completed. Once turned in, all paperwork will be kept by athletic trainer for one year- if athlete participates in more than one sport, all paperwork will be good for the entire school year excluding any expired physicals. Physicals are good for 395 days.

Athletic Eligibility Students must be in attendance for half the school day 12:30 in order to play and practice. Student/Athletes can not miss more than 13 days in the previous semester to be eligible to play. Student/Athletes must pass 3 of 4 core classes from previous semester to participate- Second semester grades used for eligibility for Fall and winter 1 sports First Semester grades used for Winter 2 and spring 60 or above is considered a passing grade

Athletic Eligibility A 7th grader cannot be 14 years of age, and an 8th grader cannot be 15 years of age before August 31st of current school year. No student/athlete may be eligible to participate at the middle school level for a period lasting longer than four consecutive semesters, beginning with the student’s entry into 7th grade.

Sportsmanship Athletes are expected to behave as ladies and gentlemen in the classroom and on the field or court. Show respect for teachers, coaches, and officials at all times. Students/Athletes are to follow all team rules as defined and outlined by coaches. Any student sent to Choices or serving ISS will not be eligible to participate in a game or practice that day. Any student given OSS will be immediately dismissed from that athletic team in season. If an athlete quits a team for any reason, they will not be allowed to try out for another team that sports season.

Spectators’ Game Conduct
We want everyone to attend games and have a good time and support each other. Any individual who chooses to berate opponents or officials, use vulgar language, or make derogatory comments WILL be asked to leave. Per NCHSAA, spectators can be removed from the event at the request of the referee. Anyone asked to leave a game or contest could possibly be suspended for the entire school year. We ask that everyone sign and adhere to a Code of Conduct.

Game Admission Costs (CCS)
Adults = $5.00 Students (ages 5 -18) = $3.00 Ages <4 = Free All students must pay admission

Communication Please follow us for practice, game, scores and up to the minute sports information on the MPMS Athletic twitter page. - All coaches are encouraged to use the REMIND app to communicate with parents. The athletic web page will also have valuable information as well.

Other players’ playing time
Additional Items Please encourage your athlete to communicate with their coach any concerns or issues. Items not discussed: Playing time Play calling Strategy Other players’ playing time We understand situations come up and you may be upset with a coach. I ask that you adhere to the 24 hour rule. Please wait 24 hours to correspond with a coach. Do not approach a coach at practice or before/after a game. These are emotional times for everyone and usually has no resolution. If a parent violates this protocol, you can and will be restricted from practices and or games. Chain of command: 1st - Coach, 2nd - AD, 3rd - Principal

Athletic Injuries/ Medical Excuses
Please let Ms. Smolick, our trainer, know about any injuries that occur. In the unfortunate event of an injury during a game, please refrain from coming onto the court or field, until summoned by the trainer or game administrator. If an athlete is excused for PE, the excuse will also apply for any sports during the season.

Concussions A concussion is a traumatic injury to the brain and presents a wide variety of signs and symptoms. Any sign/symptom after a blow to the head is a concussion until proven otherwise. New NCHSAA rule: An athlete with a suspected concussion cannot return to play until he or she is cleared by a licensed medical physician.
