Select the Complete action for Task 12.

2. Follow directions.

3.  Select the form from the list in your sponsor group.
Click to “Complete Questionnaire” using the button below. 

4. Fill in the fields and Save the form.

5. The green-colored “plus” symbol indicates that at least one form has been saved to your account.
 …and your “Saved” forms can be located in “My Content” in the category called “Evidence and Reflection by Ability” as your collection grows!
