If you do not have a current address then you will be skipped over for referrals. Schedules will calculate the number of units per month – Information will be shared at Provider Network meeting

New Data System So What? Service Order will Create SAF!
Service Schedule HDM Schedule for Delivery Days Service Changes Permanent vs. Temporary PSS vs. PSSX Constant Change Clients Service orders will require service schedule, saf’s look to schedule to determine # units in month Client calls and says caregiver is going out of town needs more service to cover this week. Service ordered is PSSX, aide only stays for 2 hours…you will be paid at the PSSX rate

PCH CLIENTS GOING TO SENIOR CENTERS
Care coordination has been advised add this to the care plan if it is appropriate for the client to attend Neither CCSP nor Medicaid NET pays for transportation to the Senior Center Other transportation funding sources may be available – call your AAA Senior Centers have the right to request payment for meals Elderly and disabled clients must be screened for senior center services – can determine if client is appropriate and can function independently. If client needs assistance or supervision someone from the ALS must accompany client Client pays for meals with the room/board payment are required as part of the ALS services; payment for meals or provision of a meal may be requested by the Sr. Center Transportation can be requested through the AAA which requires screening (most in need score) but there may be a WL; Transportation is approved based on available funding and most in need receive first.

Submit most current application –Appendix B CCSP General Manual #18 Submit evidence (not policies and procedures) of most current client satisfaction/program evaluation to support improvement of services Submit copy of HFR notification letter verifying your license covers counties for which you are seeking CCSP expansion.

ALS-F SOLICITATION May not solicit homes that are currently under contract with another provider May only submit registration request after the 30 day notice has expired Ask homes that contact you whether they are registered with another provider – must request copy of letter from current provider releasing home from contract Solicitation issues will be forwarded to DCH for review and any adverse action in accordance with Part 1 Policies and Procedures Medicaid/Peachcare for Kids, Chapter 106 E
