5. How to fill out the Turn-in DA Form 581
On TIR(residue)/TAR(live) form (Column 22, Action Code) Block 3: Unit DODAAC. See block 29 for DODAAC. Add Julien Date, Last # of current year and 3 digit date # from Julien calendar. Add 4 digit sequence number. If the unit does this it comes from their Document Ledger. If IAM does it use On TAR form use On Unserviceable TAR use Block 13a,b,c: Print name, (Must be E-5 or above on DA Form 1687 as a Requestor for the unit.) Put Date. Signature. Block 14a,b,c: Completed by IAM/DIV (Example: DPTMS AMMO Signature) Block 28: Place appropriate sticker for TIR (residue) or TAR (live). On sticker, the first open line is the Issue document #. This is found in Block 29. On sticker, the second open line is the corresponding document number for the opposite form. (on the TIR form use the Document # for the TAR form) When completing an Unserviceable TAR, use same information as the TAR form and print “UNSERVICEABLE” at the top of the document. On sticker, the third open line is the certifying E-7 or above on the DA FM 1687 or Memo, Print name and Rank. On sticker, the fourth open line the certifying Soldier signs. (This Soldier can fill both Block 13a,b,c AND the sticker). Labels through GSA: 2" x 4 1/4" labels, sticky NSN: , box of 1,000 cost $ That amount should last you a long time. Make 4 copies of each page after completed and place carbon paper between each page. Each bundle (page group) gets 2 staples at the top of the bundle.
