Who: Incoming 3rd-8th graders
When: July 8, 10 & 12th; 9:30-11:30 each day
Where: Carlyle High School Football Practice Field
Cost: $30 per camper, $20/each add’l family member
Includes a T-shirt and Rally Towel
What: A non-contact camp coached by the Carlyle High School football coaches & several high school players. 3rd-5th grade grouped together & 6th-8th grade grouped together. Cleats not required; but recommended.
To register & guarantee t-shirt size, fill out this form & return bottom portion to your child’s school office with payment before or by June 25th. On-Site registration available, but unable to guarantee shirt size.

I understand that neither Carlyle Community Unit #1, staff, nor anyone connected with the football camp will assume any responsibility for accidents, medical, dental or other expenses incurred as the result of accidents sustained during, or as a result of, any course of instruction given the applicant by the camp staff.
