3. General Information on the Tender
Objective :
To select an insurance company that can provide required (Plan 1 & 2) insurance services for our partner. For coverage area, the company should have local clinic/hospital particularly in these seven provinces namely Riau, South Sumatera, Jambi, West Kalimantan, Central Kalimantan, South Kalimantan, and Papua.

For Submission: 27 June 2019, 10:00 UTC (5 PM Jakarta time)

4. Tender Particulars
Proposal validity period : 60 days Proposal currency : IDR Duties and taxes : All Proposals shall be submitted inclusive of any direct taxes Evaluation method : Lowest priced most technically acceptable offer Payment terms : Within 30 days after receipt of the goods /services and on submission of payment documentation.

5. Evaluation Criteria: Eligibility and Formal Criteria
Bidder is eligible as defined in the document Section I : Instruction to Bidders, Article 3
Completeness of the quotations. All required questionnaires (if any), and Returnable Bidding Forms and other documentation requested under the Documents checklist have been provided and are complete.
Bidders accepts UNOPS General Conditions of Contract as specified in Section III Form A

6. Evaluation Criteria: Qualification criteria
Bidder should has experience in providing health insurance services for minimum 5 years.
The company should be able to assign one account officer to handle UNOPS account. She/He must be able to be a point of contact to any inquires/ claims/complaint.
Has hospital / clinic partner in all provinces in Indonesia.
Shall have providers (clinics/hospital) in seven provinces where UNOPS project located (Riau, Jambi, Sumatera Selatan, Kalimantan Barat, Kalimantan Tengah, Kalimantan Selatan, Papua).
Provides call center that is available at least during regular working hours, Monday – Friday.

7. Evaluation Criteria: Qualification Criteria
Company registration certificate (TDP) from Indonesia
Provider has a company that is legally registered in Indonesia and hold license from the Financial Services (OJK)

8. Evaluation Criteria: Technical Criteria
Should be able to provide the 2 types of the requested plan (Plan 1 and Plan 2) as specified in section 4 of the document : Details of the Required Service.
Should be able to cover pre-existing condition.
Should be able to provide the service for participants up to 65 years old.
Should be able to cover vitamins as doctor’s prescription.
Has web-based / mobile platform for members to access reimbursement or check claims.
