5 possible areas for joint action
Carer health and wellness review: a joint approach An in-depth and holistic approach to health and wellbeing, involving regular check ups for carers, a system which flags carers on health records and provides identification/registration cards which are used across the sector. Carer awareness across the system through a common approach to training All NHS staff and partner services - e.g. police, local authorities, housing, volunteers outside health and care roles, all social care services - receive systematic training to be ‘carer aware’. Personalised approach Health and social care providers listen, respect and value carers’ knowledge, experience and role; support wellbeing of carers and ensure availability of services, support and education. Culture change Develop a ‘kind’ culture that recognises and appropriately supports carers with an open approach to rights and responsibilities. ‘Think carer’ at all times Develop holistic approach where carers are recognised, identified and are offered opportunities which support self-care e.g. respite. Create a single information hub for carers and professionals and a carer reference group. After considering the priorities specific to carers which emerged through the STP engagement activities #YourConversation (starting Feb 2016), participants decided on five areas which they worked up using the following headings: What does good look like?; How will we get there?; What might get in the way?; and Actions to take away. @SocialEnt_UK @IVAR_UK #BHPselfcare @SocialEnt_UK @IVAR_UK #BHPselfcare

BHP Programme Update ‘Keeping well in communities’ - Exploring ways to utilise social prescribing and asset based approaches. ‘What makes us feel good – our health in our hands’ - Exploring how volunteer/community-led groups demonstrate benefit, measure impact and communicate value. ‘Building & Connecting Communities’ - Partnership improvement, scaling up learning/new ways of working. ‘Think Carer’ - Training and workforce standards for both Carers and practitioners across the STP area ‘Good life in old age’ – Exploring approaches with VCSE & citizens to co-design pilots on social prescribing, self-care, long-term conditions and mental health to support health and wellbeing in old age. ‘Social Prescribing and Care Closer to Home’ - Using emerging Care and Health Integrated Networks (CHINs) framework, and existing social prescribing initiatives, to ‘reimagine’ the role of all players in self-care at a system-wide level. For each area talk through focus, achievements so far, aims NE: Paper to the STP Prevention Board with recommendations, Durham social prescribing network, workforce HCV: developing a new collaborative approach to prevention. Research with local orgs to look at demonstrating benefit, measuring impact, communicating value (Men in Sheds) and then sharing this learning. Borough of Rochdale: building partnerships, linking up what’s already going on locally, engaging residents in self-care H&W: scrutiny group holds STP Board to account and bring the voice of carers into commissioning. Training for carers and workforce. NCL: Using the emerging Care and Health Integrated Networks (CHINs) framework, and existing social prescribing initiatives across North London, to ‘reimagine’ the role of all players (VCSE, Commissioners, residents, service users, Public Health) in self-care at a system-wide level. Mid and South Essex: ‘Breathing well – pathways for respiratory health’, Mid and South Essex: Self-care in the respiratory care pathway, working with carers, people living with respiratory problems. ‘Pathway to engagement & co-production – mental health crisis care’, Looking at ‘peer support’ to promote whole system change, community development, social prescribing, co design, prevention, digital solutions and workforce support and development.
