3 WHERE WE SERVE Each location directly serves populations with healthcare access needs. ST. JOSEPH Population 76,422 Medical – Five medical locations including a clinic serving St. Joseph’s homeless population. Dental – A dental clinic serving a high population of Medicaid and St. Joseph’s homeless population. Behavioral –One dedicated behavioral health clinic. Each clinic integrates behavioral health into their service ensuring complete healthcare options for our patients. In addition we have 5 school based LCSW’s. Pharmacy located within two of the medical clinics.

4 WHERE WE SERVE cont. RURAL NORTHWEST MISSOURI
Northwest Health has 7 medical clinics in our rural counties. LCSW’s are on site with psychiatry available by telehealth in all rural clinics with dental in Savannah and Mound City. Our rural pharmacies are located in Hamilton and Gower. We have 4 LCSW’s that provide counseling in 10 of our area schools.

Transportation was found to be a major socioeconomic factor in accessing care. Evaluation of transportation resources: St Joe Transit OATS Faith Community Taxi Challenges: Inconvenient pick up times The cost Pick up locations are inaccessible to disabled Limited or no transportation on the weekends

6 Action Plan Northwest Health Service CEO made a solution to transportation a priority. We collaborated with MRHA to partner with HealthTran to provide transportation to Buchanan and Andrew county in April of 2019. Within a few months we branched out into our rural areas. Existing staff were utilized for the initial implementation. It was apparent we needed a part time dedicated staff member. In November of 2018 we hired a transportation coordinator.
