National Launch = AGRM National Conference (June 12-15th)
On-Boarding - Phase 1
To join the program our mission must send the following information back to the Citygate:
Group Health Questionnaire
Census File
Program Deposit
On-Boarding - Phase 2 (Due September 1st)
Once we send in the requirements, we will received an electronic welcome kit with a link to a secure onboarding questionnaire & system architecture logic-tree. This will customize the program to our mission’s unique preferences and culture.
Open Enrollment = Late October/Early November
Program Effective Date = January 1st, 2019
Deposit Details:
Scenario # 1 = We DO offer healthcare today… Deposit is $159 times the # of employees currently enrolled in your healthcare program
Scenario # 2 = We DO NOT offer healthcare today… Deposit is $159 times 75% of your benefit eligible employees
