The first level of the figure is the pharmacy process, the second level is the flow diagram of tasks and the third level is the numbered subtasks that occur within each task. Subtasks are described separately for the two hospital sites. FM1, FM2 and so on indicate the subtasks where critical failure modes were identified. C1, C2 and so on indicate the subtasks that act as controls at one site for critical failure modes identified at the other site.
