Vision for the Great North Care Record
The Great North Care Record makes information appropriately accessible for individual care, self-management, planning and research Lou Elements to the programme: About connecting information to support direct patient care So individuals can access their own data and manage their care Population health management and research for improved data analytics

Our modular approach to the Great North Care Record
Lou Health Information Exchange, one of three modules that make up the Great North Care Record. Other two elements being managed as separate projects. Focus of today is on updating on the HIE Patient Engagement Platform – is about to be procured. Will also be managed by Newcastle Hospitals and will be released next year. Pop Health further behind, have been awaiting funding to mobilise the project. Have begun to establish the right networks before system is procured

GNCR Phase Two: HIE timescales
Priority is to be able to retire the NE MIG by March 2020 (end of contract for NE CCGs) with target go-live in December 2019 HIE Connections from EMIS and TPP are being tested now. Viewers for existing MIG users to be rolled out by December 2019 An IG work stream (in conjunction with SIGN) is preparing regional IG artefacts and sample/template documents for organisations to adopt/tailor.

GNCR Phase Two: HIE timescales
Work has begun on connecting the ‘first of type’ organisations and the target is for the first 5 organisations (GPs, NuTH, ST&S, CNTW, Sunderland City Council) to be connected by late 2019 Viewers for all member organisations to be deployed by December 2019 Plan for other connections for 2020 being developed
