2  Quickly identify students who are at-risk for emotional/behavior difficulties who are likely to experience school failure or drop out of school  Provide identified youth with targeted or intensive supports

3  Poor attendance  Academic Concerns  Frequent suspensions (ISS/OSS)  Consistent demonstration of externalizing/conduct behaviors  Demonstration of internalizing (emotional) behavior patterns

4  Defined as attending school less than 90% of the time during any grading period and or less than 90% cumulative attendance at any time during the school term.  Students who have greater than 5 unexcused absences in ANY grading period  Students who have greater than 18 days of cumulative unexcused absences at any time during the school term

5  Defined as failing one or more CORE subjects during any grading period

6  Defined as receiving 4 or more cumulative days of ISS/OSS at any time during the school term

7  Defined as student demonstrating disruptive behaviors several times a week or several times daily that directly impedes his/her learning or the learning of others

8  Displays Noncompliance (willful disobedience)  Displays Hyperactive/Impulsive (out-of-seat, fidgeting, frequently talking out of turn, acts without thinking) behavior  Displays Temper Tantrums  Displays Verbally or Physically aggressive behavior  Lack of remorse or guilt for misbehaving

9  Defined as student demonstrating one or more internalizing behaviors several times a week or several times a day that have persisted for greater than 4-6 weeks

10  Displays reluctance and/or refuses to interact with others  Displays a depressed mood or is sad most of the time  Appears overly worried/anxious (crying, refusal to attend school, nervous, feeling worthless  Mood changes quickly for no real apparent reason  Make frequent physical complaints (nausea, body aches, stomachaches, excessive nurse visits)  Reports strange ideas (false beliefs, excessive paranoia) and/or reports hearing voices  Attempted personal harm/self-injury and/or thoughts of suicide  Sudden decline in grades and/or personal grooming/hygiene

12 Each home teacher and/or co-teachers meet to consider each student in class to determine whether they meet any of the at-risk criterion listed on Social-Emotional-Behavioral Screening Checklist Complete the Screening Checklist ONLY for students who are believed to be demonstrating at- risk factors Complete the Screening Consideration Form to indicate students who were not believed to be currently demonstrating at-risk factors

13  Students who meet any risk criterion at the end of first and third grading period related to (a) attendance (b) failing grade(s) and/or (c) 4 or more days of ISS/OSS removals will be identified by the school counselor  Pertinent items (1-3) will be marked by school counselor or PBIS/Behavioral Support Team in each building  Screening forms will be given to given to the identified students’ English and Math teachers to complete items 4 and 5.  All completed forms shall be returned to the School Counselor  NOTE: Staff are encouraged to complete a Social-Emotional- Behavior Screening Checklist for any student who is believed to be demonstrating internalizing difficulties in the absence of other at-risk criterion
