What and Why of Early Childhood Outcomes? Who will this Impact? EXIT Question There are 2 parts to answer for EXIT: You must re-rate the child on the EXIT (1a, 2a, and 3a) and also include sources of information. Assess the child and complete a new form-do not use the entry data/form Answer the question: Has the child shown any new skills or behaviors related to [this outcome] since the last outcomes summary? EXITS Exit Data is collected on COSF Form for a student with: entry data entry data in the program at least 6 months in the program at least 6 months Dismissed from services Ages out of the PPCD program while continuing to receive SPED services
