Objective Understand the updates of the US2014 form on the list of medical services in the Summary tab of Individual Details page Record HA (Health Assessment) Results for a Normal medical case using the new US2014 form Validate the US2014 form of a Normal medical case

Prerequisites Case is Registered with a Medical Activity Case is a USRAP (refugee) or US Immigrant case Individual is assigned or scheduled for Health Assessment

Process Overview Find an Individual Record Medical Results for each service in the Health Assessment Form – Completion date is specified – Results have no significant findings – PE Result is Normal Validate the Health Assessment Form

Rules Rules are programme/workflow-specific Normal is defined by the protocol Normal is the result of the PE Medical Service Case Management Medical Tab fields are updated when the HA Status is Completed – HA Exam Date – HA Completion Date – HA Expiry Date

FIELD MAPPING TO CORE MEDICAL SERVICES Form-Service Mapping for US2014 Form

Form-Service Mapping Whenever possible, data entry fields in the form are mapped to the medical services. For medical services that are mapped, certain fields may only be updated on the form. The corresponding fields in Summary tab are disabled. Mapped fields may include Result, Completion Date, Service Provider, etc. depending on the particular medical service

Helpful Hints Appointments are not required to start recording results System updates the Medical Services in the Summary tab of the Individual Details page based on updates made in the US2014 form (including the Status) Set All No/Negative/Normal function can populate most of the data for you, but it is still best to do QA/QC
