Not Qualified for OFLA/FMLA Or exhaust OFLA/FMLA Illness requiring time off (hours or days)—5+ days Follow regular absence reporting requirements with supervisor HR request an accommodation meeting, which will require doctor’s note Meeting with supervisor, employee, and HR to identify reasonable accommodation Must have a return to work form completed by doctor to return to work Complete MESD Paid Leave Form

Workers’ Comp Injury at work (IAR and 801) Return to work with restrictions HR contact for light duty position (administrative)— must take assignment Expiration date of workers’ comp Must have a return to work form completed by doctor to return to work Complete MESD Paid Leave Form (wait period)
