2 What are we measuring? We are measuring 14 key aspects of care as listed below and measured through our outcome and process measures: Yorkshire & the Humber Shared Haemodialysis Care Programme - Measures 2 1Take Weight 2Take blood pressure & pulse 3Temperature 4Wash Hands & Clean Arm 5Collect Equipment, Clean Trolley & Open Dressing Pack 6Lining Machine 7Priming Machine 8Programming Machine & Following the Prescription 9 Insert Needles & Ensure Security Access Line & Ensure Security 10Hook Up, Bleed Out & Commence Dialysis 11Signed off as competent in dealing with arterial & venous alarms (problem solving) 12Press needle sites / place dressing 13Epoeitin injection 14Safe disposal of sharps

5 5 Balancing Measures Measuring any unintended consequences : % of patients satisfied or very satisfied with their dialysis care [score 5 or above] % of staff satisfied with providing dialysis care [score 5 or above]

6 Yorkshire & the Humber Shared Haemodialysis Care Programme - Measures 6 How the data is being captured Generically across the York and Sheffield sites, we are capturing measurements on a monthly basis. We randomly select 20 patients in total by selecting the 3 rd and 6 th patient that arrives for dialysis in a morning shift, afternoon shift or twilight shift or using a random number generator and audit the bed spaces. For example at York and it’s two satellite units at Easingwold and Harrogate, we measure across the three sites (8 patients in York, 6 patients in Easingwold and 6 patients in Harrogate) Of those shifts that we measure, we capture 100% of staff satisfaction levels. Also, at unit level they are keeping a log of individual patients and which specific tasks they are achieving to enable them to identify where improvements can be made.
