DD Network Event October 2013 – Care Co-ordination Focus Group Feedback Step 3 of the Leeds DD Care Co-ordination protocol focuses on models of care and care co-ordination DD Care Pathway Evaluation 2013 identified - The term care co-ordination is synonymous with the Care Programme Approach (CPA) People may often be accessing services that CPA doesn’t apply to e.g. drug/alcohol, voluntary sector mental health and mental health housing support When people are not under CPA the monitoring of joint working is often informal

2 - How is care co-ordinated when people are not under CPA?. If multiple agencies are involved often no one takes the lead to co-ordinate care and services can feel disjointed No standards or co-ordination across services – difficult to liaise to arrange meetings Currently very fragmented Poor planning A culture change is needed in some services to allow liaison / conversations More respect for each other

3 - How should care be co-ordinated when people are not under CPA? Better pre-planning to identify most appropriate person Joint support planning A formalised joint working plan/protocol Shared systems/ IT system available to all Must have SU at the centre and they should be as involved as possible Include those most involved i.e. families and carers

4 - How could the Dual Diagnosis Network + Lead Practitioners role be improved in the future? Opportunities to develop/map innovative practice and share information “Knowledge bank” for the role of practitioners Consult/engage with families, carers or SU Using current resources better – supporting link working and joint care Deciding what incidence of dual diagnosis within a service warrants a level 3 practitioner

MOVING FORWARD – THE NEXT YEAR Action Plan for 2014 agreed with commissioners via DD Strategy Group 3 Key actions for 2014 1.For Leeds to have a clear DD Statement of Purpose 2.To ensure Leeds has an agreed DD Competency Framework + Training Programme 3.To ensure Leeds has a robust DD strategic plan for the future

To ensure Leeds has a robust DD strategic plan for the future Systematically gather information on perceptions of key DD project stakeholders = Focus groups involving the DD Expert Reference Group, DD Working Group, lead practitioners + carers 1.Whose needs are and aren’t being met? 2.What are the barriers + solution for clients who don’t fit services? 3.If things could be done differently how would they be done / how would they work? 4.What is the role of the DD network + services supporting DD clients in the future? 5. How can the disconnection between service users, practitioners, service mangers + commissioners be overcome? Report used to inform the new mental health framework being developed for the city, re-commissioning of substance use services + action plan linked to DD statement of purpose
