4. To Make Change  Using the results from the ACIC we created a list of priorities we wish to work on  First Priority – Introducing Health Coaching to those who have attended a group program

5. Training  IEPCP had previously conducted a survey of training needs in the Eastern Region. Through their needs analysis they have created a training calender that encourages organizations to upskill staff in CDM  We had 7 staff trained in Health Coaching through the IEPCP training program  We had another 6 staff trained in Motivational Interviewing through the IEPCP training program

Practicalities  Develop a process of using TRAK – electronic health record to record the new service  Need a name for the new program  Each clinician is allocating 1 appointment slot per fortnight  After attending group clients will be offered one free Health Coaching appointment  Held a sharing session of all trained staff to use the same processes/paperwork  Recording change for Quality Assurance
