3 Program Review: Eligibility Minnesota Women – Out of state women can be eligible for Sage, but residency is required for MABC treatment coverage – Men can receive free mammography by contacting Susan G. Komen Twin Cities Race for the Cure® 952-746-1760 Women age 40 or more – There are exceptions for breast cancer screening only! (Symptoms or Family hx)

4 Program Review: Eligibility cont. Income guidelinesguidelines – Are based on 250% of FPL – Change at the end of January each year – Are based on HOUSEHOLD size and GROSS INCOME – No proof of income is required

5 Program Review: Eligibility cont. Insurance status – Uninsured- enroll them – Unmet deductibles- enroll them early in the yr. HSAs are not insurance – Co-pays- How much of a barrier is it? Your choice to offer Sage to them or not – If patient has insurance you must submit claims to insurance first Send EOB along with claim to Sage

6 Program Review: Services Office Visit (covered annually) – Clinical breast exam (CBE) and pelvic exam Mammogram (covered annually) Pap test (covered every other year) Follow-up services (when screening is abn.) – Further imaging (f-up mamm., dx mamm, br. us) – Breast surgical consult and/or biopsy – HPV testing (only for ASC-US Pap results) – colposcopy

7 Program Review: Services cont. MA-BC: For women who need Treatment – Breast & Cervical Cancer Coverage Group of MN Medical Assistance – Complete MA coverage – Patient must be diagnosed through Sage – Patient must have no insurance – Enroll through county Social Services There are specific contact people for MABC. Contact your RC for the name or check our websitewebsite Patient needs the Sage Enrollment form to apply

8 Program Review: Process Patient is enrolled at the clinic – Enrollment occurs each visit – New Sage encounter # each visit Mamm. appt. must be coordinated (between clinic and imaging facility) – Clinic should schedule the patient’s mammogram (then send Imaging form to imaging facility) – Use same encounter number as the office visit
