Goals: –Improve the availability and accessibility of local public health data to support community health assessment by counties, tribes and other local entities. –Strengthen the capacity of county councils and tribal entities to perform regular, meaningful community health assessment. –Sustain community health assessment capacity and practice in NM.

7/16/08 New Mexico Department of Health 7 Community Health Assessment Program (CHAP) Core Activities: –Provide training and technical assistance to DOH programs, counties, tribes and other local entities in the use and interpretation of public health data. –Maintain Web-based access to meaningful data and information to support community assessment activities. –Conduct population-based assessments relevant to local needs. –Develop methods to provide meaningful data for sub-county New Mexico geographies where population size permits it. –Evaluate the impacts of locally-conducted community health assessment on program and policy at the local level and within state health and human services agencies.
