Hello, My name is Angie Stroupe. I am the Benefits Manager at AFL. This presentation is to inform you of items relating to your 2013 benefits. I encourage you to share this information; along with the enrollment guide and enrollment worksheet with your spouse. Thank you for participating in this presentation.

2 Agenda Open Enrollment Carrier Change 2013 Wellness Initiatives
Benefit Summary Q&A Schedule? During this presentation we will announce a new medical carrier for 2013 and discuss Wellness Initiatives that if completed will effect your 2014 premium options.

3 Open Enrollment 2013 Open Enrollment is November 5th – 19th.
Your 2012 medical, dental and vision plan elections will rollover to 2013. Your 2012 Flexible Spending Accounts election WILL NOT rollover. If you are wish to participate in a 2013 Flexible Spending Account, you must enroll during Open Enrollment. The Medical FSA limit for 2013 is $2500 and the Dependent Care FSA limit is $5000 If you are enrolling a spouse or any dependents that you have not enrolled in the past follow-up documentation (birth certificates, marriage certificates etc.) for dependent verification will be requested after enrollment. Our Open Enrollment period is November 5th through November 19th. During this time you can access the enrollment web or call ADP to make your elections for If you do nothing you 2012 medical, dental and vision benefits will roll over for However you must make your election for the 2013 Flexible Spending Accounts (or FSA). FSA elections NEVER rollover from year to year. In compliance with the new Health Care law the medical FSA limit for 2013 is $ The dependent care FSA will remain at $5000. In addition, please keep in mind if you are enrolling a spouse or dependent that you have not enrolled in the past you will be required to provide dependent verification after enrollment. Failure to complete the audit will result in your dependent coverage being terminated and any prior claim payment being reversed.

4 Medical Plan Carrier Change
The new carrier is: Your medical benefits are not changing. This is a carrier change only. Better Network Access Better Customer Service New cards will arrive prior to December 31, 2012 A transition line has been established with BCBSSC. Members who are mid treatment can reach out to BCBSSC to ensure a smooth transition of care. Look for more information to come! Our 2013 medical carrier will be BCBSSC. Your medical benefits are not changing; only the carrier. We expect this change to result in better access to in- network providers. The use of in-network providers directly impacts your cost. In addition we expect this change to result in better customer service to our members. When you enroll by November 19th you will receive new cards prior to December 31st. If you make corrections after this date your coverage will be active, but your card will be delayed. BCBS is providing a Customer service line beginning November 1st that will allow members who are mid-treatment to reach out to BCBS. These customer service representatives are patient advocates and are committed to help assure a smooth transition of care. Look for more information to come via regarding this change.

5 Time to get serious about Wellness!
Sign Wellness Agreement Complete Health Risk Assessment Complete Biometric Screening Meet or Speak with a company provided counselor to review “red light” screening results and follow the recommended treatment plan. Attend two wellness related “Brown Baggers” educational meetings. Complete six wellness/insurance related “toolbox” educational pieces. Okay, It is time to get serious about wellness. Through August health care and RX claims for the Manufacturing Division ONLY were more than 3.7 million dollars. That is $122 PEPM more than last year. We believe prevention is our best defense against this rising cost. Through the course of 2013 we will roll out a number of health and wellness initiatives. Adherence with the initiatives during 2013 will result in your qualifying for a reduced premium differential in We believe that encouraging you to make more informed choices regarding your health through screenings and education will result in a healthier employee ultimately reducing health care cost for all of us! The initiatives include signing a wellness agreement. This agreement will confirm you intent to complete your Health Risk Assessment and Biometric Screening (or blood test). AFL will cover the cost by offering an on-site screening as we believe these items will help you know your numbers and identify areas of caution or impending danger.

Remember information from these screenings is completely confidential. Once completed the results will be loaded in your personal HIPAA protected personal health record. Areas needing attention will be noted with a yellow “caution” or red “stop” light. Employees with red lights will be required to meet or speak with a company provided nurse practitioner or Wellness Coalition health coach to discuss results and determine a treatment plan. In order to qualify for the reduced rates in 2014 you will have to comply with this plan. 6

Don’t worry, all of these items will be explained more fully beginning in January You will have until September 30th, 2013 to complete them in order to qualify for the reduced premiums in PLEASE DON’T MISS THIS OPPORTUNITY TO SAVE MONEY IN 2014!!

Open Enrollment is your time to review and make changes to your benefit elections for the upcoming calendar year. I urge you to take a closer look at your personal health and wellness needs to help determine your individual insurance needs. This is also a good time to evaluate dependent coverage. Please note that for child life insurance Dependent children are covered up to age 19 or 25. If your dependent is beyond the age of 19 and is not a full time student it is your responsibility to remove them from the plan. Finally, please update your beneficiaries. Even if you do not elect additional life insurance AFL gives you both Basic Life and Accidental Death and Dismemberment.
