The Graduate School Fellowship Office (GSFO) is to receive Forms 90 for all fellowships, both external and internal, to insure insurance eligibility. Please email to fellowships@purdue.edu if you have any questionsfellowships@purdue.edu Forms 90 are expected to be received by the GSFO 5 days prior to the PA deadline to insure timely processing for payroll CHANGES/UPDATES

All Forms 90 will begin with the start date of 8/1/12 for the 2012-13 year. If a fellowship is starting on June 1, 2012, there will need to be two Forms 90 – one for June/July and a new Form starting on 8/1/12. Summer insurance supplement will be $112.16 per month. If a student has been on a fellowship in the spring, their insurance will be paid through August and there would be $0 insurance supplement. CHANGES/UPDATES

The fellowships administered by the Graduate School will have a minimum stipend of $15,828 for the 2012-13 year. The fellowship budgets will be brought up to meet the minimum stipend. GRADUATE SCHOOL FELLOWSHIP MINIMUM STIPEND

-University Policy requires all fellows receiving external funding to complete a Conflict of Interest statement. -http://www.purdue.edu/research/vpr/rschadmin/coi/disclosure_fin_int.phphttp://www.purdue.edu/research/vpr/rschadmin/coi/disclosure_fin_int.php -Form 90s will not be released until the COI is completed, except at payroll deadline, payroll will not be held. -Business Offices and Grad Coordinators inform the fellow of the COI requirement ahead of time and indicate that once the email is received, the COI should be completed immediately. Business Office Account Management for fellowships should be the same as any other account. Since majority of Fellowship awards are incrementally funded, please review the accounts as you prepare the Form 90s for the current year for unallowable charges. Tuition and fees budgeted are not to be spent by the fellow or PI. Any remaining balance either goes to the Graduate Tuition Scholarship budget, according to the COPA hierarchy, or is returned to the sponsor. Tuition and fees are charged according to Part C of the Form 90 by the Bursar’s Office after their 6 week audit, as follows: Fall in November, spring in April, and summer in September. It is important that a student fellowship be identified as true fellowship or one that is administered as an assistantship. Fellows should receive their insurance packet at payroll orientation or from their business office PRIOR to enrollment. Full instructions are inside.

FELLOWSHIP INSURANCE ENROLLMENT Open enrollment period: August 6 th – September 8th, 2012 Students with Dependents must enroll at PUSH Student Insurance Office, Room 340 Student Only – enroll online as directed in packet Fellowship to an Assistantship Assistantship to a Fellowship 30 days to make the transition

The policy year ends 08/05/2013. Coverage can not continue past the policy year end date. Any renewal or continuation would be based on the next year’s policy’s rates and regulations.

If a fellowship policy ends and a student is no longer eligible for student insurance, but has been covered continuously for 6 months under the student insurance, the student may elect to continue the policy. Enrollment MUST occur within 14 days of the policy end date.

It is the student’s responsibility for timely renewal payment whether or not a renewal notice is received. For those renewing for second semester, renewal payment MUST be received by January 14 th so that no lapse in coverage occurs.

Insurance is required for all international students Must begin with the first day of classes (August 20, 2012) Coverage must be continuous for the duration of academic studies at Purdue. International Students must purchase the University insurance plan or submit and receive waiver approval no later than September 8, 2012. Failure to do so will cause loss of the waiver option and/or a possible $200 late fee to complete enrollment. WAIVER DEADLINE

An enrollment form for vision coverage is available in the insurance packet or in the Student Insurance office. Cost is $10.00/year. Dental insurance is NOT available for purchase by fellowship students. Dental plan is only available to employees of the University. DENTAL COVERAGE
